Book Bag Form
Fill out this form with your likes and dislikes so we know what to select for your book bag! Each bag contains two library books, a snack, a bookmark, sticker, library program information, a craft or activity, and more. The more you share, the more personalized your bag will be.
Your Teen Book Bag will be ready on the 1st of the month after 3pm. You'll have 14 days to pick up your bag. They will be located at our Circulation Desk. Ask for Jessica or Emily if you have any questions.
Filling out this form automatically signs you up for book bags through the month of February; or if you choose the six month option, through May 2025. You can stop your book bag subscription at any time by emailing teens@cadillaclibrary.org.
